We invite you to check your own state of health by completing the online questionnaire. This will help you identify your nutritional requirements, understand your symptoms and appreciate your general health needs better. For example, we often find that people are taking inappropriate supplementation or are doubling up unnecessarily.

You can download the questionnaire and print it out at your own convenience. Simply click on the file below, save and print. Then, if you would like to have your responses analysed, you can fax or post them to us for a consultation fee of £30. We will send you a personalised health profile within two working days of receipt of the form. This will advise you of any changes you should make to your dietary habits and either suggest a programme of supplements to take, or confirm that you are already on the correct path to a healthy lifestyle. If, once you've read your profile, you would like to proceed with a three month programme, we will credit you with £10 towards its cost.
